What is the person centered planning process?
Person-centered planning (PCP) is a process for selecting and organizing the services and supports that an older adult or person with a disability may need to live in the community. Most important, it is a process that is directed by the person who receives the support.

What does a person centered plan look like?
A person centred plan may include a description of the individual, past and present. It should normally, however, include a description of a vision of a more positive future for the individual (short, medium and/or long term) together with a goal-based action plan for the attainment of this more positive future.
How is developmental disability diagnosed?
The diagnosis of intellectual disability is made based on the results of cognitive abilities (intelligence) and adaptive skills (how a child functions in every day activities). A psychologist generally administers the cognitive testing. The psychologist or developmental pediatrician may also determine adaptive skills.
